NEW STUDENT REGISTRATION

YABYAKARTA ACADEMY OF PHYSIOTHERAPY

CONDITION :

1. High school diploma / vocational / MA or equivalent from all majors

2. Minimum height for men 155 cm, women 150 cm

3. In good health, not color blind, not physically disabled (which can interfere with the teaching and learning process)

4. Drug free (declared by Government Hospital, Police Doctor)

5. Passed the Written Test (Test material: Science, Mathematics, Indonesian, English) and Health Test. 

REGISTRATION INFORMATION:

1. Fill out the registration form at the AKFIS campus "YAB" Jogjakarta  (6000 stamps, 3 sheets),

2. Black and white photos: 2x3 (3 sheets), 3x4 (4 sheets), 4x6 (2 sheets),

3. Photocopy of diploma/Graduation Certificate legalized (2 sheets),

4. Photocopy of SKCK / SKKB legalized (1 sheet),

5. Photocopy of Birth Certificate (1 sheet),

6. Photocopy of KTP / Identity (1 sheet),

7. Copy of C-1 / Family Card (1 sheet),

8. Certificate of Drug Free by Government Hospital or Police Doctor (1 sheet).

REGISTRATION FEE :

The registration fee of IDR 150,000 can be paid directly to the Committee at the "YAB" Yogyakarta Physiotherapy Academy Campus or can be transferred to BNI with No. Account 0130781143 in the name of the Academy of Physiotherapy "YAB" (Proof of transfer please keep).  

* Free Written Test for prospective new students with an average report card score of 7.00 (class 3 semester I and II report cards) from SMA/SMK/MA Department of Science and Department of Health.
